Queensland's safety and workers' compensation services
This service is to be used to make application to replace an existing current Queensland high risk work (HRW) licence. If your licence is expired you will need to contact WHSQ Licensing on 1300 631 948. You can also use this service to change the name on an existing HRW licence if you require a HRW licence to be issued in the new name.

You are required to pay an application fee and the fee is not refundable.
You will require the following information to undertake the application process:
  • Your Queensland HRW Licence.
  • Details of your QGov/Google/Microsoft account. If you do not have one of these accounts you can register for a QGov account during the application process.
  • A valid email address.
  • The customer reference number (CRN) from one of the following Department of Transport and Main Roads issued licence cards i.e. Driver's Licence, Proof of age card, Marine licence indicator card or Industry authority card or a HRW product number provided by Department of Transport and Main Roads for a previous HRW application. If you do not have a relevant Department of Transport and Main Roads CRN you can still lodge your application. Workplace Health and Safety Queensland (WHSQ) will contact you to provide information about how to obtain a Department of Transport and Main Roads CRN.
  • Three evidence of identity (EOI) documents verified by a Justice of the Peace. Click here for a list of acceptable documents. If you have a Department of Transport and Main Roads issued licence card you will not be required to provide EOI documents to WHSQ.
  • A credit or debit visa card to pay the relevant fee.
  • Change of name only - You will require a change of name document, e.g. an Australian marriage or change of name certificate issued by a relevant Registrar of Births, Deaths and Marriages as well as one form of updated photographic identification or one form of category A identification issued in the new name.
    Please Note: Digital photographs and signatures held by Department of Transport and Main Roads (that meet a certain criteria) are used for the purposes of printing a HRW licence. You may be required to visit a Department of Transport and Main Roads counter to create a Department of Transport and Main Roads account or provide a new photograph and signature.
